I have three manga's where the guys look like total girls - and they USE that fact for their own purposes. I listed them in girliest order;

3.Yubisaki Milk Tea:- The guy; Ikeda - dresses up as a woman because he has some kind of fetish for dresses; and because his girlfriend is scared of MEN. Problem is; he's so damn pretty that she's kind of jealous of him... xDD
He ranks third because he doesn't naturally look like a girl. He wears a wig and BINGO he looks feminine.

2.Nosatsu Junkie:- A boy; Umi who looks like a girl, and model's for women's clothing. Tons of guys fall for him, and that's kind of how they start off the series - with a girl's "crush" turning her down saying he's in love with Umi... xDD
Ranks second due to first place.

1.Pretty Face:- The guy; a very violent "Rando" has a photo of the girl he loves in his pocket - and riding on a bus home from school - the bus crashes and totally mutilates his face and sends him into a coma for a year or so. When he wakes up; the psycho plastic surgeon has remodelled his face into his crush's face - and he looks like her twin!
(Such luck when her twin mysteriously left town two years ago... xD)
He wins because he is the HOTTEST girl I've ever seen in manga - and he's not even a real girl!
