There was a patient in thsi episode, where he had his speech center affected and he was saying everything he thought. Imagine you speak to some gal "ooo wanna do you, and wouldn't mind her too!", or "I'm sick of being married with you", etc... Though it was just a passing thought of a moment, he did love them and wanted to go through the surgery even if it meant loosing his life so he can fix this.
Though after it was fix, the damage is already done and imagine if you have to spent the rest of your life living through what you done to hurt your loved ones.
